Your Joint Clinical Care Coordinator

At Florida Hospital Celebration Health Expedition: Joint Replacement Center, we realize the many challenges that patients and their family members face when making decisions about treatment.  Our comprehensive approach to treatment strives to deliver care in a logical, connected, and timely manner so that both your medical and personal needs are met.

One of the people you’ll first meet at the FHCH Expedition: Joint Replacement Center is our Joint Care Coordinator,Rachel Rognlin, RN. She will help you with all your needs, from pre-operative education all the way through discharge and post-discharge follow-up.

Our Joint Clinical Care Coordinator will:

• Provide education throughout your joint replacement process
• Assess your needs at home, including caregiver availability
• Assist in coordinating your discharge plan to outpatient services, home or sub-acute facility
• Help you get answers to insurance questions
• Act as a personal link between you, your doctor, and others
• Coordinate questions and coordinate your hospital care with all members of the joint replacement team

Your Joint Care Coordinator will contact you after your surgeon has scheduled you for joint replacement surgery to:

• Coordinate schedules for your pre-operative total joint class and verify your medical testing appointments
• Coordinate your pre-operative care between your doctor’s office, the hospital, testing facilities and the Joint Replacement Center.
• Answer all your questions and direct you to the resources you need within the hospital.
